Occupancy Certification Form

Thank you for completing the required annual Occupancy Certification Form. There are two options to fill out this form, please follow the instructions below for whichever option you prefer.

Option 1: Print and Fill - best if you have access to a printer

1. Open the Print & Fill form by clicking here.

2. Print, fill out and sign.

3. Scan or take a picture of your filled out form

4. Attach the filled out form and a copy of a recent utility bill to an email and send to olivia@homesteadclt.org

Option 2: Fill Electronically

1. Open the Electronically Fillable form by clicking here.

2. Fill out the form and sign by typing your name(s) on the signature line.

3. Save the filled out form as a file on your computer or phone

4. Upload the saved filled out form and attach it in an email along with a copy of a recent utility bill, send the email to olivia@homesteadclt.org
